It is simple to use and available on desktop computers, Android and iOS.

I am the owner or the authorized agent for the owner of the animal described above, and I have the authority to execute this consent.
I hereby certify that I am eighteen (18) years of age or older.
I confirm that I have established a valid veterinarian-client-patient relationship for this pet, and the veterinarian has seen this pet for a complete physical exam within the last year.
I am aware that the College of Veterinarians of British Columbia (CVBC) does not have a Telemedicine Standard or Policy that establishes any special rules for telemedicine practice.
I am aware that there are risks associated with what can be safely accomplished via telemedicine and/or live chat, as there are risks to the patient when treatment decisions are being made in the absence of a proper physical exam or diagnostic testing. Such risks include, but may not be limited to misdiagnosis and inappropriate treatment decisions resulting from incomplete information.
I understand that in rare cases, a lack of access to complete medical records may result in adverse drug interactions, adverse drug reactions, allergic reactions and/or other judgement errors.
I understand that the veterinarian or technician may recommend further follow-ups either via telemedicine or in person.I understand that in very rare instances, security protocols could fail, causing a breach of privacy of medical information.
The nature of this interaction has been explained to me and I understand the risks and limitations it poses. I am aware that there are no guarantees for successful treatments based on the risks and limitations of telemedicine and/or live chat consultation.
I have been encouraged and given the opportunity to discuss any questions I may have regarding telemedicine and live chat appointments and my questions have been answered to my satisfaction. I accept that my financial obligations remain regardless of the outcome.
I acknowledge that payment is due upon completion of the telemedicine and/or live chat consult, unless otherwise stated. A credit card number or other method of payment will be provided to the clinic prior to the consult and I provide authorization for this payment to be processed once the consult has been completed. I accept that my financial obligations remain regardless of the outcome of the telemedicine and/or live chat consultation.
